In order to ensure a smooth consultation, the client needs to provide medical history information.
1. Medical Record:
History of Present Illness: outpatient medical records, discharge records, case summary, et al;
Current Status: performance status, eg daily life status, being independent; sleep; height, weight;
Past Medical History: whether the client has any medical problem in the past;
Family History: whether the client's paternal or maternal family have any medical history;
Medication List;
2. Management Plan:
Surgery/Procedure Report;
Record of Chemotherapy/Target Therapy/Immunotherapy: start and end date of treatment, medications and dose;
Record of Radiotherapy: start and end date of radiotherapy, site, doses;
Other Treatment: transplantation, etc;
3. Pathology Report: biopsy of primary or metastatic lesions, surgery, endoscopy;
4. Genetic Testing Report;
5. Radiology Report: film and report for Mammogram, X-ray, Ultrasound, CT, ECT, MRI, PET-CT, et al; recommended DICOM form of CD which can be obtained from the examination hospital;
6. Lab Reports: CBC, CMP, liver/kidney function panel, etc; please at least provide the latest reports;
7. Tumour Markers;
8. Other Related Testing Report
